Registration of New Students

2017-2018 REGISTRATION OF 1st - 5th Grade  NEW STUDENTS

The parent/guardian MUST bring the following:

•  Proof of age is required - Birth Certificate

•  Social Security Card

•  Up-to-date Alabama Certificate of Immunization

•  Withdrawal form from previous school

•  Student's most recent report card

  Copy of student's IEP, 504, etc. if applicable

•  Proof of address, such as: utility bill (water, gas, power, phone), rental 
    agreement, deed, tax papers, tag receipt, pay stub in parent/guardian's
    name

•  Legal documents pertaining to custody/guardianship, if applicable
   (must be signed by judge)

•  At least 3 current, local, WORKING telephone numbers for emergency
    contacts in the event that you cannot be reached
